Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that emphasizes healing and wellness. Nursing practices are based on evidence, current research, and established policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through supportive relationships among nursing staff and other disciplines. Responsibilities include ensuring and providing direct patient care activities on the unit. This involves nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. Provision of patient and family-centered education is essential. Care coordination and individualized treatment planning are conducted as applicable. Assessment of patient needs is performed and documented comprehensively. The RN considers the patient's culture, age, and developmental level. Identifying patient risk factors and presenting complaints is crucial. Health information is collected from family or others as necessary. Both objective and subjective data are gathered. The initial admission assessment is verified when obtained by other team members. A problem list is developed, prioritizing patient needs based on team input. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family. Patient care therapies and procedures are fully implemented within the scope of practice. The RN explains procedures, treatments, and processes to patients and families. Initiatives to alleviate patient stress and discomfort are taken. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. Tasks are delegated appropriately, and team members are overseen in their performance. Accuracy and completeness of delegated patient care are monitored. Educational needs of patients related to prioritized problems are identified and addressed. The discharge process is facilitated by executing discharge orders in a timely manner. Input into patient/nurse assignments is provided, considering staffing and patient acuity. Active participation in unit-based activities supporting organizational values is demonstrated. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is expected. A holistic perspective regarding patient care is utilized.
